We are seeking a committed and experienced support worker to support people with multiple and complex needs that require short- or long-term support with obtaining and retaining appropriate housing and relevant supports.

Haven; Home, Safe is a for-purpose organisation which connects people to housing options and support services to enable them to find and keep a place to call home.

Community Outreach Workers work within the community and with various agencies to identify and support people whose needs are often overlooked by mainstream services.  This includes those who are experiencing multiple and complex health problems and who are homeless, living in inappropriate or unsafe housing or at risk of becoming homeless. 

Community Outreach Workers will operate within a dynamic and empowering framework of assertive outreach, service linkage and support. 

The Supporting Connections Program workers undertake six main service activities. All involve working collaboratively with proprietors, staff and residents: They include conducting needs assessments and creating plans, facilitating social participation of residents in their community and coaching/mentoring proprietors and staff where necessary, to better support their residents.
