About the opportunity
We have outstanding opportunities for passionate and experienced person to work with a highly reputable and iconic community services organization. Reporting to the Program Manager this position is a Case Management role with responsibilities for supporting clients on Home Care Packages to be able set goals and access identified services. The role will be required to carry a case load.
The role will also be accountable for management of the funds and reporting requirements of the Home care Packages.
The Case manager will be required to participate in an oncall roster.

About you
We’re looking for motivated people who have a genuine interest in and commitment to working with vulnerable people in our community. You will have a tertiary qualification in Social Work, Community Services or equivalent and experience working in a aged care setting. Empathy, resilience, outstanding personal qualities and a preference for working within a collaborative team environment are qualities that will succeed in the role.
