About the role
We are currently seeking an enthusiastic Case Manager to join our Northern Outreach Team. The Northern Outreach Team provides a range of flexible, responsive and professional services for people experiencing homelessness or who are at risk of being homeless in Melbourne’s northern suburbs. We actively engage clients to assist them to gain and maintain sustainable housing and other supports. The program works in partnership with other services to keep people safe, build rapport, and engage them with the broader service system and their community. As a Case Manager, you will provide case management to clients through a range of tailored interventions to improve their quality of life. You will also assist clients to access safe, affordable and sustainable housing from which they can recover and participate in their chosen communities and networks.
About you
To be successful in this role, you will have a tertiary qualification in social work or a related human services discipline, and experience providing highquality case management to a diverse, often complex, caseload of clients. You will have a passion for working collaboratively with people experiencing homelessness and the ability to use determination, rapport building and creative problem-solving skills to deliver innovative solutions to people’s identified needs and goals.
