As part o f our Support Coordination team based in Epping, EACH is looking for an experienced Support Coordinator to join us in this rewarding high-growth area. In addition to your ability to apply customer experience principals to your work, you will have an excellent ability to foster and nurture customer relationships, a desire to help people obtain their personal wellbeing and life goals through an empowerment-based approach, and experience in clinical, educational or community-based disability, mental health or early childhood intervention services or related fields.

What you’ll be doing
Your day will see you helping clients develop the skills and confidence needed to manage their NDIS plan. Initially, your role will be to help clients get started with their NDIS plan; supporting the implementation of all supports in their NDIS plan. You will act as the key liaison point for all related services selected by the individual. Over time, however, you will play an integral role in helping each participant build their own capability to take ownership of the Support Coordination function over time.

Who you’ll be working with
The Support Coordination team is a small, but a well-established team of 8. You will primarily work collaboratively with the participant and local service providers. To avoid a conflict of interest, you will work independently of other EACH teams that provide services under NDIS.
