ABOUT the Functions:

The Multicultural Foster Care (MFC) Program provides a range of services to assist children and young people who have been found to be in need of care and protection to meet their permanent care plan needs – safety, stability, and well-being through a culturally awareness and responsive lens.

YOUR Contribution

The Case Manager will provide support to children, young people, foster carers and families which will be consistent with SSI model and internal policies.

The Case Manager will be responsible for:

  • Undertake casework including, referrals, intake, matching, assessments, planning placements, monitoring and reviews as well as support placements.
  • To develop and review Case Plans as per policies and procedures and to ensure the views and wishes of children and young people are considered.
  • Comply with SSI and with OOHC Policies and procedures in general (including DFFH best practice principles and safe standards) as well as with SSI staff responsibilities.
  • Provide information, advice, and support to foster carers whilst meeting the statutory requirements of foster care and case management.
  • Liaise/engage with Community/Support Services, in relation to referrals and reportable activity as per OOHC policies and procedures and match children and young people with foster carer as per Multi cultural Models.
  • Contribute to the cohesion and support of the team, such as participating in an After Hours On Call roster, Flexible hours and other duties as required and as requested by the Program Manager.

ABOUT YOU:

EXPERIENCE & QUALIFICATIONS

You are a passionate Case Manager – OOHC or child protection and experienced in providing casework support to children, young people, foster carers and their birth families within the Multicultural Cultural Foster Care Program; the aim is to achieve permanency care outcomes.

  • Tertiary Education in Social work or relevant field (Human Services, Psychology or Social Work, Welfare)
  • Experience working with culturally & linguistically diverse communities/families.
  • Ability to speak, read and write in English, Excellent written and oral communication skills
  • Demonstrated casework experience, OOHC, family services and/or child protection
  • Demonstrated understanding of the Department Family Fairness and Housing with a particular emphasis on the Child Protection function
  • Knowledge of the relevant legislation (Child Youth and Families Act 2005 and Disability Act 2006)
  • Knowledge of theories relevant to the role and ability to implement intervention strategies
  • Well-developed analytical and organisational skills
  • Negotiating strategy, techniques and skills
  • Computer literate with well-developed MS Office skills are essential
  • Travel is an essential requirement for this role
