About the Program:

SSI has been successful to deliver the National Disability Insurance Scheme Partners in the Community Program (PITC), in the South West Syd ney and Sydney districts, until June 2023. The program provides for Local Area Coordination (LAC) services to assist Participants in developing or reviewing support plans as well as extensive work in the community to promote inclusion and opportunities for people to access mainstream service.

The PITC Program in SSI will cover the fourteen local government areas in South Western Sydney and Sydney Metro Area’s with a team of over 200 people including Local Area Coordinators and other management, administration and support staff located at a range of sites across each area.

The LAC Planner will work with the participant (and where appropriate their family and other informal supports) to determine what reasonable and necessary funded supports and services will assist the participant to achieve their goals. They will gather the relevant information, build and develop the plan for NDIA approval.

The LAC Planner will also work with participants to make changes to their plan through the plan review process which covers scheduled and unscheduled reviews.

Duties and responsibilities:

  • Gather information to make informed funding decisions based on reasonable and necessary criteria.
  • Work closely with participants to identify what current and future supports are required to make progress with a person’s goals and aspirations and enable better outcomes
  • Scheduling appointments for the LAC team and NDIS planning meeting.
  • Provide information to participants and their representatives to identify their goals and aspirations.
  • Providing relevant in-language information, advice on Disability and NDIS.
  • Conduct planning conversations with participants to create plans to support the goal of living an ordinary life
  • Linking people with participant readiness activities, liaising with the Access and Support team to
  • confirm capacity building workshops and community engagement dates and locations

ABOUT YOU

You possess proven interpersonal and communication skills with the ability to build effective relationships and liaise across all levels both internally, externally and with people from diverse backgrounds including Aboriginal and Torres Strait Islander and CALD communities. You have demonstrated ability to work with limited supervision, manage a variety of tasks and display personal drive and integrity. All LAC Planners need to have the willingness to travel and work flexible hours and in a range of locations across areas and regions as required.

  • Bachelor equivalent qualification in relevant or related field e.g. Psychology, Social Work or Allied Health or Qualification in disability/community services with equivalent experience working with people with complex needs
  • Demonstrated experience in work closely with participants to identify what current and future supports are required to make progress with a person’s goals and aspirations.
  • Conduct planning conversations with participants to create plans to support the goal of living an ordinary life
  • Providing relevant in-language information, advice on Disability and NDIS
