About the role

Merri Health – Diabetes Nurse Educator

  • 0.5 EFT; Permanent Part Time position
  • Site: Various Merri Health Sites
  • Join an established multi-disciplinary team, who strive to help our clients reach their desired outcomes

Your new role

You will be working as part of an established multidisciplinary team to provide diabetes education and advice to clients through diabetic assessment, appropriate care plans and providing evidenced based care. This role is funded by Community Health and will require credentialing with ADEA or working towards credentialing.

As the Diabetes Nurse educator in the Prevention of Chronic Illness Care Team you will be responsible for:

  • Assessing adult clients diagnosed with type 1 or type 2 Diabetes Mellitus including those newly diagnosed, on insulin or other medications and managed by lifestyle interventions
  • Provide education on diabetes and self-management principles including blood glucose monitoring, use of medication, healthy eating principles, management of hypoglycemia or hyperglycemia and activity levels
  • Initiating and titrating insulin and other injectable agents in accordance with DNE insulin work instruction
  • Participation in planning, designing and implementation of group programs that operate across different sites of Merri Health

 

What you’ll need to succeed

To be successful with Merri you will go above and beyond to achieve great results for our clients, the organisation and yourself. We’re looking for individuals with:

  • Qualification in Nursing and Post Graduate Certificate of Diabetes Education with or working towards credentialing by ADEA and current registration with AHPRA
  • Demonstrated experience in providing diabetes education and assisting client in self-management of diabetes
  • Experience in working autonomously and within multi-displinary teams
  • Experience in Diabetes Nurse Educator clinical service program development, planning and evaluation
