About the role
VincentCare Victoria is currently looking for a passionate and experienced individual to join the Community Connections program! As the Case Manager, you will work closely with various stakeholders to provide support to marginalised individuals experiencing crisis or homelessness. You will be utilising short-term and medium-term case management strategies to stabilise a client’s crisis, identify and implement seamless pathways to ongoing supports, creating linkages, advocating and supporting clients to access mainstream and specialist services, particularly housing and health services.
We have a lot of work to do in this space. In your first year you will work on:
- Providing assertive outreach to engage with clients around their unmet health needs and to link the with the Community Connections Program;
- Utilising persistent and assertive engagement techniques to establish and maintain a robust professional relationship with clients;
- Managing a case load in an Outreach case management capacity;
- And plenty of other interesting initiatives…
About you
You are an appropriately qualified professional with experience in providing high level assessments, case planning and support response to people experiencing homelessness.
You are well known for your collaborative, integrated and client centred approach to providing tailored support to your clients. You are thorough and detailed in your practice of completing ongoing assessments, case planning and review, and making appropriate referrals. These skills have enabled you to successfully stabilise the client’s crisis, create pathways to ongoing supports, and facilitate access to appropriate and sustainable housing.
You are passionate about working closely with a client to enable and empower.
What We Are Looking For:
- Experience working in an assertive outreach capacity to engage with clients and to link them with support;
- Experience in case management practice and navigating service systems with and for clients;
- Demonstrated understanding of the range of needs and issues affecting people who are homeless or at risk of homelessness;
- Highly developed communication and relationship management skills;
- Demonstrated ability to work independently and as a member of a team;
- A strong alignment to the values of VincentCare.
