An increase in client numbers through our Home Care Package (HCP) Program means that we are looking to welcome a new Care Advisor (Case Manager) to join our great team.

Australian Multicultural Community Services (AMCS) is a “for-purpose” / not-for-profit organisation established by a group of visionary community leaders who were concerned about migrant welfare and wanted to ensure that non-English speaking migrants had the same access to needed services and opportunities. Fast forward 39 years and we’re proud to have continued implementing our founding members’ initial vision. Our extensive services cover every aspect of wrap-around supports as needed to keep our clients safe, connected with their communities and comfortable at home as much as possible during their senior years.

We have an exciting opportunity for a dynamic, enthusiastic, caring, compassionate and experienced health professional who is committed to making a positive and significant difference to the lives of older people who live in the community.

 

The Care Advisor position with AMCS is one with purpose and we support our staff by providing a respectful and encouraging environment that drives a collaborative approach with all stakeholders to ensure the highest quality support can be provided.

 

In the Care Advisor role:

  • You will act as the key contact and advocate for our clients and their representatives
  • You will assess, monitor and review clients’ needs using a person focussed and reablement approach and develop goal-directed care plans that put the client in the centre of all we do
  • You will ensure that service provision occurs in a consistent and quality driven manner and is compliant with HCP guidelines
  • You will encourage and drive strong partnerships internally and externally and work collaboratively as a member of the AMCS team
  • You will apply your cultural competency skills across our proud multicultural organisation reflecting the AMCS values at all times

 

 

Qualifications, experience and requirements

  • Tertiary qualifications in nursing, health, allied health, social science or welfare discipline, or in a related field
  • Outstanding interpersonal, communication, networking and negotiation skills and the ability to engage with a broad and diverse range of people
  • Experience in managing a busy case load in a community setting using a person centred approach
  • We have a proud history of supporting both clients and employees from migrant communities therefore an additional language other than English will be a distinct advantage
  • A working knowledge of Home Care Packages and the Aged Care Quality Standards, community resources and service networks for older Australians
  • Exceptional customer service focus
  • Appropriate knowledge and experience using the Microsoft Office suite
  • A current Victorian Drivers Licence
  • COVID-19 vaccinations (1&2)
  • Consent for police check
