About the role

 CatholicCare Diocese of Broken Bay is seeking an enthusiastic Care Coordinator and subject matter expert in Aged Care to ensure optimal customer service and individualised care is provided to our clients receiving our services.

This is a 3-day (24 hrs/wk), Permanent Part-Time position with a view to Full-Time. There is a requirement to travel between our office based in West Gosford and client homes based across our diocese for assessments and care management.

 

 Duties

  • Provide effective case management to our clients and be flexible and responsive to ensure we can offer high quality and continued support to clients changing care needs.
  • To provide exceptional customer service across all external and internal stakeholders.
  • To support a personalised on boarding process of new clients for all our Senior Living (Aged Care) program/s, which includes guiding them on the eligibility process through My Aged Care to ensuring they are provided the best possible care.
  • Provide guidance, support, and training to our team to improve our quality of care.
  • Collaboration with internal/external stakeholders, including strengthening our relationships within the Aged Care industry, while building referral pathways.
  • Build on our positive brand name within the Community, with aim to increase referrals to our services.

 

Requirements

 Certificate IV in Aged Care, Community Services or related field OR equivalent experience/education

  • Experience working with Seniors, including those living with Dementia
  • Good knowledge of the Aged Care system and services
  • Demonstrated experience in client budget monitoring and care management.
  • NSW Driver’s Licence and access to an insured vehicle
  • Access to personal smart phone
  • Current Senior First Aid or willingness to complete this training
  • Willingness to complete a Criminal History Check
  • Applicants will be required to disclose your COVID-19 vaccination status as part of your application for employment. You will also be required to provide further evidence of vaccination status or medical contraindication certificate as part of your recruitment /interview process.
