Be Recruitment is recruiting for a Case Manager to join our client’s Intensive Therapeutic Care Service (ITC) based in Tamworth, NSW. 

Intensive Therapeutic Care (ITC) is a service system that helps children and young people who are recovering from the most severe forms of trauma, neglect, abuse or adversity.

ITC is for children and young people over 12 years with complex needs who are either unable to be supported in foster care or require specialised and intensive support to maintain stability in their care arrangements.


Key Responsibilities
 

  • Drive the implementation and achievement of the case plan goals and plans for the permanency of children and young people.
  • Develop and maintain functional and supportive relationships with children and young people in out-of-home care as well as their families in order to promote positive outcomes.
  • Support and monitor to ensure the case planning process and assist them to implement the strategies outlined in the case plan
  • Manage communication with key stakeholders, internally and externally in order to achieve case plan goals

 

Skills & Experience

  • Bachelor’s degree in Social Work, Social Science, Psychology or Human Services or working towards this
  • Previous case management experience required (ITC, OOHC, PSP, Child Protection etc.)
  • Vaccinated or exempt from vaccination against COVID-19
  • Demonstrated understanding of current casework practice for children and young people
  • An understanding of the permanency support program
  • Previous experience in working with individuals, families or groups
  • Strong written communication, client/stakeholder engagement, and mediation skills
  • Current Australian driver’s license
