About the role
Outreach Case Manager – Older Persons High Rise Support Program
As an Outreach Case Manager with the Older Persons High Rise Support Program (OPHRSP), you will provide vital support to older individuals living in public housing estates in Northcote and Brunswick. This full-time role focuses on short-term case management, connecting clients with essential services, and helping them sustain their housing to avoid premature entry into aged care or hospital settings due to unmet health needs or social isolation. You’ll also engage in community development activities, fostering connections and enhancing residents’ quality of life. Join a dedicated team making a real difference in the lives of older community members.
We are seeking a candidate with a qualification in community services, social work, or a related field, and experience working with vulnerable populations, particularly older adults. The ideal candidate will possess strong advocacy and case management skills, a solid understanding of aged care, disability, and public housing systems, and a commitment to supporting diverse communities, including CALD, Aboriginal and Torres Strait Islander, and LGBTIQ individuals. Excellent communication, interpersonal, and organisational skills are essential, along with the ability to work collaboratively within a team environment.
