For the past 30 years, our proven Model of Care has successfully transformed the lives of over 1,000 young people using a range of evidence-based strategies. We now educate and support Foster Carers in the same way, so that they too can take care of some of Australia’s most vulnerable children.

About the role

The Therapeutic Case Manager role within Lighthouse Foundation’s Children and Family Services offers a unique opportunity to provide both therapeutic support and case management to vulnerable children and young people aged 0–18 years. This includes those living with their parents, family or carers, and those transitioning from care to independence. Working within a multidisciplinary team, the Therapeutic Case Manager will deliver tailored, outreach-based support that promotes stability, wellbeing, and long-term positive outcomes. The role involves the development and implementation of care plans, as well as the delivery of individualised therapeutic and community interventions.  

This role is primarily based in our South Hub (Keysborough) with some hours at our Richmond Youth Resource Centre when required.

WHAT DO I NEED TO BE SUCCESSFUL?

First and foremost, interpersonal skills are a must, with a level of maturity and understanding needed when working with those who use and support our services.

From a professional standpoint you will require;

  • Experience and knowledge in best practices responses to responding to children and young people who shave experienced trauma, as well as supporting their caregivers 
  • An understanding of attachment and trauma informed care
  • Strong ability to engage, motivate and support young people with complex needs who have an experience of out of home care or homelessness 
  • Great relationship building skills which enables you to meet young people’s needs and achieve meaningful goals 
  • Demonstrated capacity to work flexibly and have the ability to manage competing demands.
  • Excellent communication, verbal and written skills
  • Strong organisational, time management, and administration skills 

Other key skills which would be desirable;

  • Demonstrated skills in delivering case management
  • Skills and experience in supporting caregivers  

For this role you will please note you will also need:

  • Tertiary Qualification in relevant/associated field
  • Current driver’s license
  • Valid first aid certificate (or willingness to obtain one)
  • Criminal Records check
  • Current Working with Children check
