About the program

The Maribyrnong Community Residential Facility (MCRF) provides an accommodation option for men exiting prison, with a strong focus on transitional support, managing risk and community safety. The facility provides accommodation for men post-release and those on parole. To be eligible, men will need to have no immediate access to suitable housing on release from prison.

The program is a partnership between Corrections Victoria and Jesuit Social Services. Jesuit Social Services delivers pre- and post-release case management and transition support to men at the facility; addressing their individual and complex transitional needs through a responsive, tailored and flexible approach to facilitate community reintegration and reduce recidivism.

The aim of the program is to support eligible individuals to effectively reintegrate back into the community on their release from custody. The program supports people to address their individual and complex transitional needs through a responsive, tailored and flexible support approach to facilitate community reintegration and reduce re-offending. The key objectives of the program are to:

  • Implement an individualised transitional plan for participants which addresses key needs to reduce re-offending and ensures continuity of care
  • Provide assertive outreach and practical assistance to participants to successfully reintegrate into the community
  • Assist participants to address the underlying causes of their offending and achieve positive outcomes in key intervention areas.

About the role

The Case Manager role will be responsible for providing outreach-based case management to participants exiting custody with multiple and complex needs who are deemed suitable to reside at the Maribyrnong Community Residential Facility. This role will be based onsite at the Maribyrnong Community Residential Facility (MCRF) with outreach services delivered across metropolitan Melbourne.  This role requires working closely with key stakeholders in particular Corrections Victoria, Community Correctional Services (CCS), prisons and other external organisations. The role includes frequent travel to Victoria’s ‘mens’ prisons and includes provision of day of release transport from regional locations. We are looking to recruit an enthusiastic and experienced Case Manager who is able to provide holistic transitional support to participants exiting custody.

Responsibilities for this role include:

  • Work closely with the Senior Practitioner to support day-to-day operations of the Reconnect program
  • Manage a caseload of participants with multiple and complex needs
  • Ability to work without supervision in the community in an outreach capacity
  • Support participants to develop individually tailored case plans that are goal-focused and participant-led

A contract can be offered until June 2026 with possibility of extension

About you

This role will suit a candidate with the following skills and experience:

  • Experience working effectively with people involved in the criminal justice system and a strong understanding and empathy for the challenges facing individuals as they transition from custody to the community.
  • Experience working with men to address the underlying issues that contribute to offending behaviour and a knowledge of specialist services and community based support/s
  • Highly developed verbal and written communication skills; and stakeholder management.
  • An understanding of risk management and the capacity to assess for risk and respond appropriately.
  • Good practice frameworks and strong relational and interpersonal skills
  • Strong understanding of working within a compliance framework
