Care Manager (Locum – Community Services) Southern Region

Care Managers in our team have a vital role which is complex and pivotal in the effective care delivery to the community. We are a positive team of committed individuals and SCCV Southern Region Community Services have goals which revolve around and support our clients.

Our Care Managers liaise with clients, carers, local health services and existing service providers, Aged Care Assessment Services (ACAS) and other health professionals. We work closely with older people and their families to conduct initial assessments, develop care plans and communicate with referring agencies to enable people to continue to live at home. We monitor, review and adjust as required in consultation with client/carer and other relevant parties.

Along with tertiary qualifications (mandatory) in Social or Health Sciences or equivalent, our Care Manager will have exceptional communication skills, good time management, IT skills, a cooperative approach and an efficient work style.
