Intake and Assessment Worker OnTRACC
- Full time 2 year fixed term position.
- Based inRockdale ,NSW

The Australian Community Support Organisation (ACSO) is a non-government , not for profit agency which provides accommodation , support , assessment and treatment services for people who are experiencing significant disadvantage.
The NSW Department of Justice via NSW Corrective Services has adopted a new model for the delivery of transitional services to individuals exiting prison effective as of 1st July 2016. The key objectives of the program are to:
- Intake and Assessment: Assess and identify the offenders criminogenic needs and personal strengths. Commence service planning, case plan and risk management plan, prior implement transition and risk management.
- Dual Diagnosis Assessment and Bridging Support: Targeted support and clinical consultation for offenders with a dual diagnosis( I.E. Mental illness and substance use in the critical period between initial assessment and first treatment appointment.)
- Jobs, Housing and Life Skills: Actively support offenders’ engagement and achievement of the Corrective Services NSW identified support service domains.
- Aftercare: Flexible and responsive aftercare approach. Aim is to support offenders once episode of support has concluded to maintain an offence free lifestyle in the community.
- Outcome focused: Reducing the rate that offenders return to custody.
The Intake and Assessment worker will assess every referral to the program and identify their criminogenic needs, risks and personal strengths. Under the guidance of the Coordinator, the Intake and Assessment Worker will commence service planning, develop the case plan to be provided by staff and they will develop the risk management plan. They will work closely with On TRACC Staff and Corrective Services NSW to ensure that services delivered achieve targets, meet KPI’s and contractual outcomes.The Intake and Assessment Worker will support the Coordinator in the day-to-day intake functions for On TRACC.
