About the role

- Enrolled or Registered Nurse experience essential

This is an excellent opportunity to join a supportive team within an innovative organisation, and utilise your case management experience in delivering home care packages.
This is a permanent fulltime position which would suit a motivated and client focused Case Manager with either Registered or Enrolled Nurse experience. You are customer focused and confident in the delivery of services, experienced in aged care reform and home care packages. This role will see you travelling to consumers’ homes, providing case management and support for level 2, 3, & 4 home care packages within the Logan River Valley and Brisbane South region.
Role Features
- A primary focus to build strong relationships with clients & their families to make empowering and positive decisions
- The development of effective support plans
- To undertake effective leadership of the support workers including coordinating & allocating workloads according to consumer’s needs, staff skills & experience
- Manage budgets & 24 hour rostering
- Staff selection & performance reviews
Selection Criteria
- Minimum 2 years Case Manager experience within Aged Care, Community or Healthcare Setting (Home care packages advantageous)
- AHPRA Enrolled or Registered Nurse with solid experience in managing wound care, continence, pain/palliative management, medication administration and stoma/PEG care
- Experience managing budgets & rostering
- Excellent customer service skills with customer focused attitude
- Proven expertise in assessments, care plans, case management & coordination
