Based in Stawell, approximately 235kms west of Melbourne,Grampians Community Health (GCH) is a not-for profit Community Health Centre that has been providing coordinated health and community services to people within the Central Grampians and Wimmera sub regions since 1986. Their multi-disciplinary approach to psychosocial health aligns with a Social Model of Health philosophy and aims to promote healthy living in the Grampians whilst also improving the standard of living.
About the Opportunity
Grampians Community Health have an opportunity for a dedicatedRisk Assessment and Management Panel (RAMP) Coordinatorto join their team in either Stawell or Horsham, on a part-time basis.
Working as a key component of the Victorian Strengthening Risk Management (SRM) Program, you’ll be contributing to the effective operation of the RAMP, in order to reduce risk and harm to women and children at serious and imminent risk, to reduce the potential for perpetrators to pose a serious and imminent risk to women and children, and to strengthen the capacity of the service system to achieve the above two goals.
Some of your key responsibilities will include:
- Facilitating referrals to the RAMP and seeking relevant information to inform suitability of referrals for consideration by the RAMP;
- Identifying parties and individuals to participate in the RAMP process;
- Ensuring that relevant information to participate is sent to participating RAMP members;
- Preparing information for RAMP members and providing administrative support including keeping records/minutes and collecting data;
- Supporting the Chairs in monitoring and following up action plans, and monitoring and reporting on RAMP performance; and
- Developing and maintaining relationships with relevant agencies and promoting the work of the RAMP.
