Our programs are delivered within a framework of strengths-based, recovery-oriented practice, consumer, carer and community participation and codesign and a whole of health approach to care. We achieve consistent, high quality recovery-oriented services using the Collaborative Recovery Model (CRM) and Recovery Star and ensure competency in trauma informed care, family centred practice delivered in a client directed purchasing environment.

The Youth Residential Rehabilitation programs support young people (aged 16-25 who have a diagnosed mental illness) for up to 12 months and are located in Essendon (7 beds), Reservoir (8 beds) and St. Albans (9 beds).

Cohealth’s Youth Residential Support staff work closely as a team to provide flexible, intensive recovery-based support to young people who are experiencing mental health challenges. This is achieved by utilising both individual and group opportunities within a Collaborative Recovery Model. Working collaboratively with families, clinical supports and other internal and external service providers to ensure a holistic, best practice approach is maintained.

 

As our Youth Residential Support Worker your responsibilities will include:

  • Providing recovery-oriented services to young people, utilising the CRM and Recovery Star models and coaching principles. Including identifying needs, aspirations and goals in the development and implementation of client-directed, strengths-based care plans;
  • Providing support to young people in developing and maintaining social, domestic, personal care, recreation and other skills needed for independent living and self-management in the community;
  • Facilitating access to community resources, peer leadership initiatives and services; and
  • Developing and maintaining collaborative relationships – particularly with clinical and community services partners and the family and significant others of young people within the program.

About You

To be considered for these exciting opportunities you’re required to possess:

  • Tertiary qualification and/or commensurate work experience in fields related to mental health, youth or health;
  • Excellent knowledge of and/or experience in working with young people within a youth developmental framework;
  • Experience in and/or understanding of residential service provision in health or community services, particularly mental health;
  • A detailed understanding of Community Heath and the social model of health principles, mental health service delivery models of care underpinned by the principles of family centred and inclusive practice, trauma informed practice and approach and the collaborative recovery model;
  • Ability to communicate and model a vision that generates respect, enthusiasm and commitment; and
  • A lived experience of mental illness and recovery as a consumer, family member or carer will be highly regarded.
