About the Team:
Cohealth’s support coordination team focuses on providing quality support coordination to Melbourne’s most marginalized communities. We specialise in supporting people experiencing mental illness; homelessness and unstable housing; a disability or engaged in the criminal justice system or drug or alcohol use; or are from a culturally or linguistically diverse background. As we look to continue the expansion of our team, an opportunity has arisen for us to bring on board a talented, committed NDIS support coordinator.
Your new role:
A large part of the role will consist of coordinating access to services identified in participant’s NDIS funded packages, as well as supporting participants to strengthen their abilities to connect with relevant supports, to develop and maintain support relationships and to increase their capacity to independently navigate through the challenges involved in coordinating their support requirements. Building on your local knowledge of the available services, seeking to encourage a person centred, strengths based and/or recovery orientated approach by these services.
