The HARP Complex Care Program is a partnership between The Royal Melbourne Hospital (RMH), cohealth, Merri Health, and Bolton Clarke. We aim to improve patient outcomes, provide integrated seamless care within and across hospital and community sectors; reduce avoidable hospital admissions, Emergency Department (ED) presentations and ensure equitable access to health care.

This position is responsible for providing comprehensive assessment and targeted interventions for persons who frequently present to RMH with a range of complex psychosocial needs. The position also co-ordinates safe transition and continuity of care, by conducting comprehensive assessments, coordinating specialist clinical and community services to ensure sustainable services can support clients in the community.

Are you often commended on your interpersonal skills, do you demonstrate integrity and reliability, with the ability to fosters open honest professional communication? Ideally your experience, qualifications and skills for the role should include:

  • Current registration with relevant Australian Health Practitioners Regulation Agency (AHPRA), or Eligibility for membership to relevant professional association;
  • Experience within outreach service provision and conducting risk assessments with complex care clients;
  • Demonstrated experience in comprehensive care needs assessment and care planning; and
  • Knowledge of the social model of health, and various funding streams through Aged Care and/or various community services.
