Community Services Coordinator

Spinal Cord Injuries Australia is passionate about continually improving its service offerings and driving innovation to provide maximum opportunity for those whose lives are affected by traumatic injury. It is therefore essential that all activities are carried out with a view to supporting the organisation in the achievement of its Vision: “A society without barriers for people with spinal cord injury”

The Community Services Coordinator will assist clients with planning for services, and provide services (such as connection and coordination of services and supports, primarily under the NDIS) to people with a spinal cord injury (or similar physical disabilities), their families, carers and friends.

Main Responsibilities:

  • Provide case management to identified customers
  • Provide direct support coordination and plan management/financial intermediary services for our NDIS customers
  • Attend NDIS planning meetings and liaise with NDIS planners as required
  • Create and complete goal setting and outcome based plans for customers
  • Collaborate with customers, families, providers, medical practitioners and other stakeholders.
  • Understanding of the NDIS and NDIS participant plans

  
To be suitable for this role you will have:-
  

  • Case management / case worker experience
  • Industry relevant formal qualifications or training, such as a certificate III or IV in Disability or Community services
  • Understanding of the NDIS and NDIS participant plans
  • Strong analytical, problem solving and conceptual skills
  • Good financial acumen including the ability to review and explain financial statements
  • Proven ability to work positively with people to reach their individual goals
  • Excellent documentation skills in particular case records and incident planning and reporting
  • Have a person centred and outcome focused approach
  • The ability to meet KPI’s in relation to billable hours
  • Unrestricted NSW driver’s licence.
  • All SCIA staff must obtain and maintain a clear Working with Vulnerable People check in the relevant state jurisdiction and also at a national level.
