Your new role
As a Support Cooridnator you will be part of a highly motivated team of staff, utilizing a person centered approach you will work closely with individual NDIS participants and their carers to:
- Enable them to find and choose the right supports and services to meet their needs
- Increase their capacity to understand the NDIS and make the most of their funding
- Link them with appropriate mainstream services
- Establish and maintain links with their local community and turn their goals into achievements
What you’ll need to succeed
To be successful with Merri you will go above and beyond to achieve great results for our clients, the organisation and yourself. We’re looking for individuals with:
- A relevant health or disability related Qualification (Social Work, Nursing or other health related profession) or Certificate IV In Disability
- Experience working with people with physical and or intellectual disability or working with vulnerable people with complex needs
- Understanding in planning, case management, coordination and reviewing for people with complex needs
- Advanced communication and interpersonal skills
- Ability to work in an activity driven fee for service market
