About the role
The Enhanced Care Clinician will work across all aspects of headspace sites providing brief episodic support to young people struggling to engage with the health system. The Clinician works both within the local headspace clinical team and across the region interfacing with other enhanced care Clinicians as needed to provide seamless service transitions.
Reporting to the Manager, Youth Enhanced Services, the key component of this role is the provision of flexible evidence based support resulting in increased capacity to engage and support young people with complex needs in an enhanced primary care setting.
Activities and deliverables
Full-time fixed term position available until 30 June 2020
The Enhanced Care Clinician will:
- Engage in and deliver shared care with the Access team, medical, collocated and contracted staff to support the provision of youth-friendly early intervention services at headspace.
- Provide a range of brief episodic therapeutic interventions and care coordination as needed including assessment and short-term outreach.
- Undertake intake, assessment and referrals processes for more complex and ‘higher risk’ young people and assist others in the team to do so.
- Provide clinical support to a caseload of complex and higher risk young people which may include providing community outreach.
- Promote the headspace program, communicate regularly with, build and maintain collaborative working relationships with health and welfare agencies and the general community.
