About the role
Could you support 15-25 year olds who are experiencing homelessness? Bring your client-centred approach to our Youth Services team and make a difference to young people in need.
The Creating Connections program is a state wide government initiative to address the multiple and complex needs faced by Youth who are homeless. Grounded in the Youth Homelessness Action Plan (stage 2), the Creating Connections program has been implemented across Victoria to provide a more comprehensive approach to Youth homelessness through elements of intensive case management, partnerships, and client centred practices. This program is located within the Ovens and Murray Area, and has partnering agencies within Wangaratta, Seymour and Shepparton. The Creating Connections Case Manager will be based in Junction’s Wodonga office, and will provide an outreach service to young people aged 15-25 years old within Wodonga and outlying areas. The Creating Connections Case Manager is also responsible for managing the Fostering Independence Property (“FIPS”). This property supports young adults aged 15-18 years old to provide a safe, stable and supportive living environment in which young people are given the opportunity to further develop their independent living skills and re-engage in the community.
