CatholicCare NT is a not-for-profit organization, providing social services and programs to individuals, couples, families, children, groups, schools, and agencies across the Northern Territory.
The National Aged Care Advocacy Program (NACAP) is free, independent, and confidential. The aim of the service is to facilitate advocacy support and information, outreach and education, community development and support to consumers or potential consumers of Australian Government subsidized Aged Care services, their representatives, and their families in Tennant Creek.
About the role
As a Case Worker, you will be responsible for providing Advocacy support and information, outreach, and education to participants and aged care providers in order for the program to meet its objectives. This position has a particular focus on the rights of the elderly, that they are involved in decisions affecting their lives, and that they can self-advocate for their needs in a residential care or home care setting.
This role is responsible to provide:
- providing advocacy support and information
- providing outreach and education activities and education sessions
- developing case plans which identify short, medium, and long-term case management goals
What we are looking for
- Relevant qualification in Aged Care Work or similar or in Community Services or equivalent with substantial experience in a similar role.
- Strong interpersonal and communication skills, ability to engage with and manage complex situations, involving homelessness, poverty, mental health, family violence, alcohol, and other drug misuse.
- Advocate clients and work effectively with stakeholders to meet the objectives of the Program
- Demonstrated cultural competency, particularly in working with Indigenous people.
